Application of Criminal Record Certificate

 By the request of the Ministry of Foreign Affairs, the police is issuing "Criminal Record Certificate", or often referred to as "Police Certificate", upon applications from those who are required by foreign embassies/consular's offices.
 At the Okayama Police, applications can be made for those who currently has his/her residence registration or alien registration within the Okayama prefecture, or those who has had his/her last residence or last alien registration certificate registered within the Okayama prefecture.

Necessary Documents(1)
Japanese nationals living in Japan:
1 Your valid original passport
2 Any one of the following documents is necessary:
 a A driver's license
 b An official copy of Resident's card ("Juminhyo") (issued within the last six months)
 c A Certificate of Items Mentioned in Resident's Card ("Juminhyo Kisaijiko Shomeisho")(issued within the last six months)
 d A Basic Resident Register Card ("Juki Card")

Foreign nationals living in Japan:
1 Your valid original passport
2 Any one of the following documents is necessary:
 a Residence Card("Zairyu Card")
 b Special Permanent Resident Certificate("Tokubetsu Eijusha Shomeisho")
 c An official copy of Resident's card ("Juminhyo") (issued within the last six months)

Japanese nationals living abroad:
1 Your valid original passport
2 Any one of the following documents is necessary:
 a An official copy of Resident's card ("Juminhyo") (issued within the last six months)
 b A Certificate of Items Mentioned in Resident's Card (issued within the last six months)
 c A certificate of revoking residence registration or an authentic copy of KOSEKI-NO-FUHYO, the additional card to the family registration (issued within the last six months)
3 Documents that verify his/her current address such as valid driver's license issued by foreign government.

Foreign nationals living abroad:
1 Your valid original passport
2 A document that shows you lived in Okayama
 An official copy of Alien registration certificate(issued within the last six months)
3 Documents that verify his/her current address such as valid driver's license issued by foreign government.

Necessary Documents(2)
Any one of the following documents is necessary:
1 Documents that state your reason of applying Criminal Record Certificate such as documents issued by foreign government that state obtaining Criminal Record Certificate are required.
2 Completed Visa application forms and/or others.
 
Processing fee:
750 yen by Okayama Prefectural revenue stamps (Okayama-ken shunyu-shoshi) is required.

Important: Please contact us in advance for verification of necessary procedures, for, depending on the country or its reason, your application may not be immediately accepted.
 
Others:
* Your personal appearance is required.
* It should take approx. 10 business days for processing.
* As for issuance, you or your representative may receive the certificate (your representative needs to bring a letter of proxy authorized by you, along with your representative's identification.)
* If you are a Japanese national living abroad, application can also be made at Japanese embassy/consular's office of the country you reside, however, double applications, both abroad and in Japan, are not acceptable.

Directions:
* 15 minutes' walk from JR Okayama Station
* Bus transportation: Take Okaden-bus lines from JR Okayama Station Bus Terminal, bound for "Kokuritsu-byoin"
* Others: get off at "Kosenkyo-higashi" bus stop. It takes 3(three)minutes' walk.
* If you are coming by car, please be careful of numerous one-way streets.
